    Responsibilities & Qualifications

    Position Description

    • The Health Coach will be responsible for contacting participants that are eligible for the Care Management program for enrollment, initial interview and providing ongoing necessary health coaching to reduce or eliminate high-risk behaviors
    • The Health Coach will be responsible for developing a health coaching relationship with members and assisting the member through the process of actively improving management of their chronic condition by providing support, encouragement and education  
    • The Health Coach may perform community outreach activities to assist in locating members and engage them in Care Management services.  Activities include, but are not limited to:
      • Assist in coordinating members’ relationships with multiple service systems. This may include activities such as:
        • Assist members to obtain a Medical Home and providing instruction on appropriate use of the Medical Home
        • Assist members in overcoming transportation barriers to obtaining medical care
        • Inform members about available community resources and assist members in utilizing the resources
      • Support and coach members to improve management of their chronic condition. This may include activities such as:
        • Present health education in a culturally appropriate format. 
        • Provide social support to improve members’ adherence to medical treatment plan
        • Encourage adoption of habits that are conducive to improved management of their chronic condition
        • Develop an individualized plan including goals and plan of action
        • Serve as a resource to other Care Management team members 

    Location / Work Environment

    • Independent, home office based environment.  Must have access to a high-speed internet connection and be able to comply with work-at-home agent requirements.

    Position Qualifications

     

    • Minimum of two years' experience as a Health Coach
    • Bilingual language skills preferred (Spanish and English)
    • Experience with personal computer using Windows operating system
    • Exhibits effective communication skills for working with members, their care givers and their healthcare providers
    • Demonstrates the ability to motivate members to make critical health changes in their lives
    • Demonstrates a commitment to members and a willingness to go above and beyond normal responsibilities to provide the best care possible
    • Exhibits the ability to resolve conflicts with members and overcome their objections about their health and behaviors
    • Strong organizational skills

    Education and Professional Certifications

     

    • Must have current Health Coach certification from ahealth coach training program in health promotion, chronic condition management or health education (e.g. WellCoaches® or equivalent).
    • Bachelor’s degree preferred
