    Responsibilities & Qualifications

    Position Purpose:  The Disease Management Registered Nurse provides disease management nursing services and care coordination to high risk, high cost Medicaid patients in a geographical region, or telephonically to a commercial population.  The RN provides education, counseling and on-demand information to patients who have enrolled in a condition specific program.  As needed, the RN will provide program education and coordinate care needs with the patient’s Primary Care Provider.  Services are performed in person or telephonically and interventions are recorded in the Population Care Management (PCM) application.

     

    Essential Functions: 

    1. Using the Nursing Process, provides nursing care within the standards of the state Nurse Practice Act
    2. Provides care to individuals or defined patient populations through the use of telecommunications in accordance with computer-based algorithms, protocols, or guidelines
    3. Using the defined process, adheres to nursing standards, company policies and shared principles when providing clinical assessments and health education
    4. Uses the nursing process to identify patient care needs, risk and safety issues, educational opportunities, and appropriate health care referrals
    5. Processes calls based on Quality Management guidelines
    6. Demonstrates compliance with all product specific performance metrics
    7. Regular, reliable attendance and timeliness

     

    Job Requirements:

     

    Education: Associates or Bachelor’s degree in Nursing. 

    • Registered nurse responsible and accountable for safe clinical practice, knowledgeable of customers and regulatory expectations in the clinical area.
    • Current RN license in good standing
    • For patient and provider facing staff, must have: - current CPR certification  - current PPD  - current valid driver’s license and reliable transportation with  required level of automobile liability insurance coverage

    Experience:

    • Minimum of 3 years of clinical experience.
    • Good communication and telephone skills.
    • Basic computer skills.

    • Ability to communicate via telephone with members/customers and provide good customer service.
    • Good listening skills
    • Demonstrate professional oral and written communication
    • Applies ICARE principles to all aspects of the job
    • Demonstrates sound clinical judgment in accordance with the Nurse Practice Acts
    • Makes decisions that adhere to policy using basic critical thinking skills
    • Self-motivated
    • Able to ask for assistance and guidance
