    Responsibilities & Qualifications

    Purpose of this position:  To provide intensive social work support in identified cases of members who meet enrollment criteria, focusing on members who are high and/or moderate risk.  This includes a variety of support to members with serious or severe physical illnesses, developmental disabilities, experiencing EOL issues, caregiver burnout or complex psych/social barriers that interfere with obtaining maximum benefits from qualifying programs.  


    Position Description

    • Reviews identified and/or referred High Cost/High Risk (HCHR) cases to formulate individualized plans of care
    • Builds relationships with members through individual or group conferences to assist members and families in understanding and implementing care plans to improve or restore members to maximum medical, social and mental health
    • Provides expertise in coordination and assistance with processing applications to members who qualify for alternative benefit options
    • Provides ongoing coordination of community and governmental agency access to services to mitigate members' barriers to optimal health and well-being
    • Assists in improving self-care and self-advocacy
    • Works collaboratively to develop and carry out a comprehensive care plan to provide maximum benefit for each member.
    • Is a resource to other team members
    • Identifies, develops and maintains relationships with community, health and welfare agencies and state groups that are involved in the care of vulnerable populations and who may be resources to the program population


    Position Qualifications

    • Three plus years of experience in clinical social work
    • Current unrestricted license within the work state
    • Certified Social Worker in Health Care (C-SWHC) preferred
    • Thorough knowledge of social work practice and the application to case management including complex case management and behavioral health
    • Understanding of family and group dynamics; familiarity with change behavior techniques
    • Knowledge of government and private organizations, resources in the community, laws, regulations and policies that govern clinical social work
    • Skill in establishing rapport with members, families, and local resources (medical, social, civic, legal, religious organizations)
    • Ability to communicate and execute work effectively
    • Preferably a member of the local community

    Education and Professional Certifications/Requirements

    • Bachelor's Degree or Master's Degree in Social Work
    • Current PPD.
    • Current valid driver’s license and required level of automobile liability insurance coverage.

    Salary is in the low to mid $20,000.00 range with great benefits.
